Primary Care - Retinal Screening
Each Practice will need to be briefed on the introduction of the service and have access to training and other implementation support.
1st Retinal Screen provides these implementation services. We have developed a Practice Overview Document and we brief all relevant
members of the Practice on the DR Screening Programme.
Once the Practice has confirmed their participation, there are two key areas of interest in relation to the DR Screening Programme. The Practice needs to:
- Determine which of their patients are to be included within the screening programme, via the electronic-Prior Notification List (e-PNL) review; and
- Following screening, download the results that are sent to them and have access to the images in a timely manner.

1st Retinal Screen provides this function on-line using its web-based systems
The Practice may access this service from any PC connected to NHSnet/N3.
A key feature of the way we deliver the service is that only minimal resources
are needed to be expended by the Practice. Our data load facilities ensures that
all patients who may need to be included within the retinal screening programme are
identified, all the Practice then needs to do is review the electronic list and select
if they wish the patient to be invited or not. The Practice may choose to invite now or
in the month that the patient is due (based on their screening history). A patient can be postponed if it is not appropriate
to attend until a later date. If you choose to invite a patient now, a letter is issued the next day. The Practice
may update the status of their patients at any time.
- Accessing the Results from the DR Screening Programme will be available on-line as soon as grading is complete and prior to the patient receiving their result letter. A future enhancement will see the results automatically loaded into the patient's records on the Clinical system.

Secondary Care - Retinal Screening
The Ophthalmologist receiving the referral from the DR Screening Programme will have access to CDRIntell
to review the results and other relevant data contained with the patient record. They will also be
provided with access to the retinal images and be offered the opportunity to comment on the
appropriateness of the referral.

CDRIntell Supporting Clinicians with Primary Care
There are a number of audit and reporting services, which take data from General Practice Clinical Systems
and present reports back to the Practice. Sometimes these reports are set in context of other Practice's
data - benchmarking. 1st Retinal Screen's approach is different, we actively look for meaning in the
data that is recorded on the systems. For example, we map all relevant data, not just diagnosis (as others do),
but findings, laboratory test results, prescription data, symptoms, etc to determine those that are of interest.
Combining this data with hospital data sources allows us to support General Practice by:
- Notifying the Practice of missing discharge information (the value of this should not be underestimated in reducing re-admissions).
- Notifying the Practice of missing findings, laboratory test results, clinical examinations, etc from outpatient appointments - leading to an improved performance against the QoF (this activity doesn't need to be undertaken by the Practice).
- Maintaining an up to date electronic record for your patients with chronic disease, so that the Clinician can minimise duplication and better target those patients not being looked after by either sector.
- Comprehensive clinical governance reporting.
- Comprehensive Case Management Finding - identifying those patients who would benefit the most from proactive management.

CDRIntell Supporting Clinicians with Secondary Care
Providing secondary care clinicians and their departments with the latest information about their patients
with chronic disease ensures that treatment is provided more effectively. For example, access to the
latest set of prescription data and laboratory test data will enable the Consultant to make improved
and more timely decisions regarding treatment regimes, drug titration, etc.
With the ever-increasing demands on Secondary Care, the system can support programmes for improving discharge
arrangements by identifying those that are stabilised and offering them as candidates for discharge.
Similarly General Practice will be supported in identifying those patients who should perhaps be referred.
