Patient Consent Form
1st Retinal Screen has developed a Patient Consent Form for the purpose of obtaining informed written consent to the screening procedure and for the use of data about the patient  used by the local healthcare purchasers and providers in the provision of improved care to patients with Diabetes. The Patient Consent Form is issued with the Appointment Confirmation Letter and is mail merged with the patient's details for convenience. The retinal screening procedure will not be carried out without patient consent. Should the patient refuse to provide their consent, the patient’s information will remain in a “sealed envelope” and will only be accessible to the patient’s GP Practice.
